If you are applying for a green card from inside the United States, your application needs a completed Form I-693, the Report of Immigration Medical Examination and Vaccination Record. Here is what happens at the exam, so you can arrive prepared and leave with your sealed form as quickly as possible.

Who Can Do the Exam?

Only a doctor designated by USCIS as a civil surgeon can complete and sign Form I-693. A report from any other doctor is not accepted. You can check that a doctor is designated with the civil surgeon locator on the USCIS website.

What Happens at the Visits

Most exams take two short visits:

  • First visit: your history is reviewed and blood is drawn for the required lab tests.
  • Second visit: once the results are back, you have the physical exam, your vaccination record is reviewed and any missing vaccines are given.

"Bring every vaccination record you have, from any country. Each one we can count is a shot you may not need to repeat."

Which Tests Are Required?

The tests depend on your age and follow the CDC's instructions for civil surgeons. They usually include:

  • A tuberculosis blood test (IGRA) for applicants aged 2 and older.
  • A blood test for syphilis for applicants aged 18 and older.
  • A urine test for gonorrhea for applicants aged 18 to 24.

Vaccines

The civil surgeon compares your records with the vaccines the CDC requires for your age and gives any that are missing, unless there is a medical reason not to.

What to Bring

  • A government-issued photo ID, such as your passport.
  • All your vaccination records.
  • A list of your current medications and any medical records about ongoing conditions.

Key Takeaway

Keep the envelope sealed

Your civil surgeon gives you the completed Form I-693 in a sealed envelope. Do not open it: USCIS only accepts the form in its sealed envelope, and you submit it together with your Form I-485.
